Argentina reported 317 adverse cases in those vaccinated with Sputnik V

The Ministry of Health informed this Friday that 317 adverse events have been reported after the application of 32,013 doses of Sputnik V vaccine, in the first two days of the national vaccination campaign against covid-19 that Argentina started last Tuesday.

These adverse events have been reported until 23.59 local time last December 30th to the Integrated Health Information System of Argentina (SIISA), according to the first vaccine safety surveillance report published today by the Ministry of Health.

"44.2% of the reported ESAVIs (adverse events) correspond to people with fever, headache and/or myalgia initiated 6 to 8 hours after the vaccination," he explained.

These events are related to the Sputnik V vaccine, added the report, which details that within the expected reactions, the following can occur: pain at the injection site, hyperemia, swelling. 

And that within the systemic reactions, the following can occur: short-lived flu-like syndrome that starts within 24 to 48 hours (characterized by chills, fever, arthralgia, myalgia, asthenia, general malaise, headache) or less frequent gastrointestinal symptoms (nausea, dyspepsia, decreased appetite). 

"These reactions have an average duration of 24 hours," he explained.

Argentina began last Tuesday the process of simultaneous vaccination throughout the country, with the batch of 300,000 doses of the Russian vaccine Sputnik V received on Thursday of the previous week, which is applied to health personnel.

The report published today informed that 18 of the 24 jurisdictions of the country have reported adverse events to SIISA.

"99.3% of the reported events were mild and moderate, not requiring hospitalization and evolving with full recovery," he added.

BEGINNING OF THE CAMPAIGN

With some mistrust due to the lack of public dissemination of scientific information on the Russian vaccine, Sputnik V, Argentina applied the first dose, which should be completed after 21 days with the second component.

The target population at this stage is health personnel of large urban agglomerations, health personnel of closed intensive care units and laboratory personnel, who handle the virus to make diagnoses.

Argentina has committed 51 million doses, according to the contracts the country signed with AstraZeneca for 22.4 million doses, with the Russian Investment Fund for 20 million doses of Sputnik V vaccine and through the World Health Organization's COVAX Mechanism for 9 million doses. In addition, the country continues to negotiate to acquire vaccines from Pfizer, Sinovac and Sinopharm laboratories.

Today the only vaccine available is the first dose of the Russian vaccine. The President, Alberto Fernandez, has said that Russia will deliver the second 300,000 doses in the first days of January, at the end of January 4 million of the first dose and 1 million of the second, and in February the remaining amount to finish vaccinating 10 million people.

The intention is to continue in a next phase with vaccination of adults over 70 years of age; then adults between 60 and 69 years of age; the armed forces, security and prison services; teachers; and then other strategic populations.

The vaccine should be administered in 2 doses, a minimum of 28 days apart.
Operationally, it is recommended that the second dose of both vaccines should be routinely scheduled between four and 12 weeks after the first dose. This will allow more people to benefit from the protection provided from the first dose during the roll out phase. Longer term protection will then be provided by the second dose.
If an interval longer than the recommended interval is left between doses, the second dose should still be given (preferably using the same vaccine as was given for the first dose if possible).

Therefore, every effort should be made to determine which vaccine the individual received and to complete with the same vaccine.
For individuals who started the schedule and who attend for vaccination at a site where the same vaccine is not available, or if the first product received is unknown, it is reasonable to offer one dose of the locally available product to complete the schedule. This option is preferred if the individual is likely to be at immediate high risk or is considered unlikely to attend again. In these circumstances, as both the vaccines are based on the spike protein, it is likely the second dose will help to boost the response to the first dose.
For this reason, until additional information becomes available, further doses would not then be required.

Neki neće Fajzerovu, ali nema smislenog odgovora zašto. Neki hoće, ali ne odmah, neki neće jer je to zavera. Da se ne lažemo, rok za izjašnjavanje je bio prektatak, a ove se radi u smenama, tako da mnogi nisu ni bili u ova 2 dana na poslu.

Dat je ekstremno kratak rok za izjašnjavanje, kod mene ljudi rade u smenama. mnogi nisu bili na poslu. Pošto smo u četvrtak poslali traženi izveštaj, još je ljudi dolazilo, tako da se do kraja mog radnog vremena prijavilo nešto preko 100 ljudi. Onda smo se dogovorili da im pošaljemo još jedan izveštaj u utorak ili sredu, a po mojoj proceni tada će biti oko 200 potpisa. Mnogi su dolazili i pitali, da li mogu da se izjasne za neku drugu vakcinu, najčešće SputnjikV, tako da će sigurno biti i više zainteresovanih kada u opticaju bude više vrsta vakcina.

That situation in the UK appears to be one of the biggest factors driving the approval and rollout, and I see their point: this vaccine is indeed better than nothing, one shot for more people is likely to be better than two-shots-for-some, and it looks like they’re going to need all the help they can get. But “better than nothing” is a rough place to be. So what do we know about the efficacy of a single shot of the Oxford/AZ vaccine, and about the effect of waiting for a second one?

All I can say is that attempts to answer those questions land you immediately in a confusing mess. It’s a mess made worse by AstraZeneca, whose CEO has made statements about the vaccine’s efficacy that are not (so far) backed up by actual numbers. If you’d like me to name a major drug company that’s going to come out of this pandemic looking worse, it’s them. Anyway, as you’ll recall, initially there was a hint that a lower first dose followed by a standard second dose might be more protective overall (although I don’t think the evidence for that is very strong at all, considering the statistical spread in the data). But now there’s a report that increased efficacy might be driven by an even longer wait between the two doses. I don’t find that evidence very compelling, either (we’re getting into some pretty small subgroups by this point, and that is always a dangerous area to draw conclusions from). And if you’re going to leave people walking around with a half dose at first, or a full dose but with a longer wait for the second one, it makes the question above even more crucial: how protective is one dose?
